Youth Registration Form 2026/2027

Start Date: Tuesday September 22, 2026

To be completed once per school year, by a legal parent/guardian of the Youth attending Immanuel’s Unlimited Soul Youth Group. A Registration Form must be completed for each youth attending Youth Group on a regular basis, or before a Special Event.
